Reamed intramedullary nailing versus circular frame external fixation for segmental tibial fractures (STIFF-F): a mixed methods feasibility study

چکیده

Abstract Background Segmental tibial fractures are in two or more areas of the diaphysis resulting a separate intercalary segment bone. Surgical fixation is recommended for patients with segmental as non-operative treatment outcomes poor. The most common surgical interventions intramedullary nailing (IMN) and circular frame external (CFEF), but evidence about which better poor quality. An adequately powered randomised controlled trial (RCT) to determine optimum required. STIFF-F aimed assess feasibility multicentre RCT comparing IMN CFEF fracture. Methods was mixed-methods study comprising pilot conducted at six UK Major Trauma Centres, qualitative interviews drawing on Phenomenology an online survey rehabilitation. primary outcome recruitment rate. Patients, 16 years over, fracture (open closed) deemed suitable were eligible participate. Randomisation stratified by site using random permuted blocks varying sizes. Participant assessor blinding not possible. Interviews undertaken their experience injury, treatment, recovery participation. Staff interviewed identify contextual factors affecting processes, pathway. developed understand rehabilitation context treatments. Results Eleven screened three recruited RCT. Nineteen staff four participated interviews, 11 physiotherapists responded survey. This found following: (i) rarer than anticipated, (ii) complexity setup times surgeon preferences impeded recruitment, (iii) from challenging, protocols inconsistent (iv) despite difficulty recruiting, valued this research question strived find way forward. Conclusion proposed feasible. highlighted recruiting complex rare injury over short time period. Trial registration registered International Standard Randomised Controlled Trials Number Registry: ISRCTN11229660

Treatment of Open Pediatric Tibial Fractures by External Fixation Versus Flexible Intramedullary Nailing: A Comparative Study

BACKGROUND Tibial fractures are the third most common pediatric long-bone fracture after forearm and femoral fractures. Approximately 50% of pediatric tibial fractures occur in the distal third of the tibia. This is followed by midshaft tibial fractures (39%), and least commonly, the proximal third of the tibia is involved. Tibial fractures in the skeletally immature patient can usually be trea...

A meta-analysis of reamed versus unreamed intramedullary nailing for the treatment of closed tibial fractures.

Controversy exists regarding the clinical outcomes of reamed vs unreamed intramedullary nailing in the treatment of closed tibial fractures. This study assessed the effects of reamed vs unreamed intramedullary nailing for closed tibial fractures. The authors searched PubMed, EMBASE, BIOSIS, and the Cochrane Controlled Trials Register for randomized and quasi-randomized controlled clinical trial...

Flexible intramedullary nailing versus external fixation of paediatric femoral fractures.

Treatment outcomes were compared in two groups of children with femoral diaphyseal fractures which were treated with external fixation (20 fractures) or flexible intramedullary nailing (20 fractures). These 40 children were between 5.4 to 14.1 years of age. The duration of the operation averaged 52 minutes for the external fixator compared with 70 minutes for the flexible nail group. The time t...
